~ Case Management ~
HELP’s Case Managers provide guidance and assistance to individuals and families living in these area shelters: Interfaith Family Shelter, Shalom House, and the YWCA. They assist clients in obtaining an income and/or permanent or temporary housing. This system allows the shelter managers to work on other equally important goals such as child care, connection to the mental health and/or drug and alcohol recovery support systems, family unification, etc.
Once placed in shelter, clients are assigned a Case Manager to work with them throughout their shelter stay. Upon completion of an intake, Case Managers are required to meet with clients within 48 hours of placement. During this initial session, Case Managers aid clients in developing an individualized goal plan designed to break down the steps involved in resolving their situations. Clients are also required to develop a “Plan B”. Plan B, simply put is, "if all else fails what will the client do?"
Each week, Case Managers monitor and measure individual progress and assist clients in assessing if and when Plan B will be needed. Furthermore, Case Managers meet with the shelter managers each week to conduct a client review and to brainstorm resources. The desired outcome resulting from all of these activities is permanent and/or temporary housing.
Case Managers also work woth street homeless, those who are about to become homeless, and "non-traditional shelter" residents. The Case Manager's role is to either prevent homelessness or help secure more stable housing.
